After an injury a few years ago, his thumb had been severly injured, and afterward, the bone didn't heal quite right. It was like the thumb's joints were skipped and the individual pieces of bone healed together. He was barely able to work anymore, and had changed the way that he used his axe in a way that ended up just hurting his other fingers. Sadly, it had been too long for Eiro to 'save' this through surgery, for example by re-breaking the bone and healing it differently. The woodcutter's soul was already adapted to the new thumb far too much.
